😖 Reporting an injury

It’s important that you report all injuries to us at practice or on game day.

Even if it’s only something small, it’s important this paperwork is process on the day, and ensures you remain eligible to make an insurance claim.

These reports are saved internally, and are used to support you in the event you need to make a claim.

To complete an injury form by hand or digital, and return it via:

  1. Hand it back in to one of your Your current committee members.
  2. Email a photo to [email protected]

Your role in your claim

It is up to you (or your parents / guardian) to submit a claim.

As the claim forms request a lot of incredibly specific and private information (as does every insurer), they’re best completed by you.

SCBA’s role in your claim

As part of your insurance claim, we will submit your injury report via GameDay, and link it to your profile.
